	To enter the contest, fill out the official entry form below. We accept only  fully completed entry forms. Only one entry per person per month is  allowed, but please feel free to enter each month. Multiple monthly  submissions will be deleted.

	Contest begins 12:00am on the first of the month, and ends on the last day  of the month at 12:00 midnight. Winners will be selected in a random drawing.
